    Location: Williamstown, KY 
    Position Type: W2 | Part-Time

    Practice Setting: Skilled Nursing Facilities (Post-Acute & Long-Term Care)

    Position Overview

    Telos Health Systems is seeking a compassionate and motivated Attending Physician (MD/DO) to provide comprehensive on-site primary care services to residents in skilled nursing facilities.

    Clinical Responsibilities

    • Provide comprehensive primary care services for residents in skilled nursing facilities
    • Evaluate, diagnose, and manage acute and chronic medical conditions
    • Perform admission evaluations, routine follow-up visits, and medically necessary assessments
    • Evaluate and manage changes in patient condition
    • Collaborate with nursing staff, rehabilitation therapists, specialists, and facility leadership
    • Supervise and collaborate with Nurse Practitioners and Physician Assistants as appropriate
    • Participate in interdisciplinary care planning and quality improvement initiatives
    • Complete timely and accurate documentation within the company EHR

    Call Responsibilities

    • Participate in a structured and manageable call rotation
    • Address urgent clinical concerns through telehealth
    • Telehealth encounters provide additional earning opportunities
    • Supported processes designed to maintain a reasonable call volume

    Compensation

    • Competitive productivity-based compensation utilizing a Work RVU (wRVU) model
    • Earnings are directly tied to clinical productivity and patient care services provided
    • Stable earning potential supported by consistent facility census and predictable rounding schedules
    • Additional compensation opportunities through billable telehealth encounters

    Qualifications

    • MD or DO from an accredited medical school
    • Board Certified or Board Eligible in:
      • Internal Medicine
      • Family Medicine
      • Geriatrics
      • Emergency Medicine
    • Active Kentucky medical license (or ability to obtain)
    • Active DEA registration (or ability to obtain)
    • Interest in skilled nursing, post-acute, and long-term care medicine
    • Excellent communication and collaborative care skills
