Job Summary
This position provides routine patient care to promote patient comfort and contribute
to the overall patient plan of care. This position is responsible for on-going patient data
collection and documentation in the medical record
Essential functions:
- Under the supervision of the nursing staff accepts responsibility for the care of clients as assigned on the shift worked
- Performs direct patient care including, but not limited to: assisting clients with daily living activities such as bathing, grooming, feeding, changing, vital signs, age appropriate play
- Responsible for making the client's formula, as instructed by the nurse on duty
- Performs basic living activities; repositioning of clients, transferring patient as directed by nurse, and utilizes proper body mechanics when performing patient transfers
- Participates in client related activities; such as performing therapies issued by the healthcare team which includes; massaging client, range of motion exercises, proper orthotic placement
- Demonstrates safe usage of assistive medical devices such as Hoyer lifts, walkers, crutches, splints, etc.
- Carries out duties as assigned to maintain a clean and sanitary environment; stocking supplies, household cleaning duties as assigned
- Abides by all Health Care Alternatives policies and procedures
- Responsible for knowing and following policy relating to absences, tardiness and work breaks
- Must perform duties following safety standards required for this position including but not limited to use of PPE's
- Must attend all training sessions required by the company
- Appearance should be professional and complies with agency dress code
- Maintain a cooperative manner towards client/family and all members of the healthcare team

Minimum Qualifications:
- Certified Nursing Assistant certification in good standing with the Arizona State Board of Nursing, is preferred but not required
- High school diploma, GED or equivalent experience
- Does not require experience, however, previous experience in a similar setting, nursing home or in a special education classroom will be an advantage
- Current CPR
- First Aid Card required unless applicant is a nursing student
- Class 1 fingerprint clearance card
- Additional qualifications/certifications needed within 3 months of employment (Article 9, Prevention and Support, Trach/Vent class/training)
Job Requirements:
- Must be at least 18 years of age.
- Must abide by company policies and procedures
- Must be able to maintain a class 1 fingerprint clearance card.
- Must be capable of performing a 50-pound weight lifting test performed as part of the required physical exam
