Travel PT Home Health jobs in Glendale, AZ are available for 13-week assignments, Monday through Friday from 8am to 5pm, with 40 hours per week. You must have home health experience and an active AZ license. Your responsibilities include evaluating patient mobility, developing individualized treatment plans, providing therapeutic exercises, gait training, and mobility education, and documenting care in Homecare Homebase EMR. You will cover Glendale, Peoria, Sun City, and Surprise, and receive mileage reimbursement at $0.72 per mile 1.
