Behavior Care Worker (Part Time Available)

We are seeking a Home Health Care Worker to become a part of our team! You will provide routine healthcare to patients at the patient's home.

Responsibilities:

• Provide routine individualized care to persons with disabilities
• Monitor and report changes in patient health status
• Provide personal care, such as bathing and dressing patients
• Provide specialized care that focuses on non-medical services for members
• Record notes on me member progress on daily goals and the results
• Teach family members ways to care for patient

Qualifications:

• Experience in hospice, home health, or other related field service
• Must have reliable transportation and clean MVR
• Positive and patient demeanor
• Ability to build rapport with patients
• Ability to handle physical workload
• Excellent written and verbal communication skills
• Quality Customer service to members

Requirements:

• Must be have or be willing to obtain a fingerprint clearance card
• Stated Required Training must be completed within 30 days of hire
• Must be 18 years of age and older to be considered for employment
• Background is required for this role
