Schedule:Part-Time Nights | 24 hours/week | Monday, Wednesday & Thursday | 2:00 PM – 10:30 PM

As a Behavioral Health Social Worker/Therapist, you'll provide compassionate, evidence-based therapeutic care while helping patients and families navigate life's most challenging moments. Working as part of a collaborative behavioral health team, you'll play a vital role in promoting healing, recovery, and hope through individualized treatment and coordinated care.

Conduct comprehensive behavioral health assessments, including psychosocial evaluations and risk assessments.
Develop individualized treatment plans with measurable therapeutic goals based on patient needs and clinical findings.
Provide individual, family, and group therapy utilizing evidence-based therapeutic interventions.
Evaluate patient progress and modify treatment plans based on ongoing clinical assessments.
Document assessments, treatment interventions, patient progress, and outcomes accurately while maintaining patient confidentiality.
Collaborate with physicians, nurses, case managers, and interdisciplinary treatment team members to develop and coordinate patient care plans.
Participate in multidisciplinary treatment team meetings by providing clinical recommendations and patient progress updates.
Develop comprehensive discharge plans in collaboration with patients, families, and the interdisciplinary team, ensuring appropriate outpatient appointments and community resources are arranged prior to discharge.
Coordinate referrals to appropriate internal services and community behavioral health resources to promote continuity of care.
Identify and appropriately intervene in cases involving suspected abuse or neglect, demonstrating sensitivity to age-specific and patient-specific concerns.
Demonstrate knowledge of the involuntary commitment process and assist with appropriate interventions when indicated.
Maintain therapeutic relationships with patients and families while promoting recovery, resilience, and emotional well-being.
Support departmental quality initiatives and participate in process improvement activities.
Perform other duties as assigned.
Knowledge, Skills & Abilities
Knowledge of behavioral health assessment, treatment planning, and therapeutic interventions
Ability to conduct comprehensive psychosocial and risk assessments
Ability to develop individualized treatment plans and coordinate appropriate referrals
Knowledge of individual, family, and group therapy techniques
Ability to collaborate effectively within an interdisciplinary treatment team
Knowledge of behavioral health documentation standards and patient confidentiality requirements
Strong clinical judgment and critical thinking skills
Ability to recognize and appropriately respond to crisis situations, including suspected abuse or neglect
Knowledge of discharge planning and community behavioral health resources
Knowledge of the involuntary commitment process
Excellent verbal and written communication skills
Ability to establish therapeutic relationships with patients and families from diverse backgrounds
Strong organizational, time management, and documentation skills
Basic computer proficiency, including Microsoft Office and electronic medical record systems
Ability to read, write, and communicate effectively in conversational English
Commitment to continuous learning, professional development, and process improvement
Education
Master's Degree — Required
Field of Study:Social Work, Psychology, Counseling, or a related behavioral health field
Experience
Two years of relevant behavioral health experience — Required
One year of experience in a healthcare setting — Preferred
Licensure & Certifications
Licensed Bachelor Social Worker (LBSW)
Basic Life Support (BLS) — Preferred
