Job Title: Behavioral Health Care Coordinator
Location:
Illinois (Must reside in IL) Remote + field Visits in Will and Kendall Counties (Joliet, Oswego, and surrounding areas)
Duration: 6 Months Contract (Possibility of extension)

Pay Rate: $39-40/hr

Position Summary
We are seeking a licensed healthcare professional to provide care coordination services to members through telephonic and field-based care management. This role focuses on conducting health assessments, developing care plans, coordinating healthcare services, educating members, and connecting them with appropriate healthcare and community resources.
The position requires regular interaction with members through phone outreach and in-person visits in homes and facilities to support complex and specialty populations.

Key Responsibilities

Care Coordination & Member Support

  • Serve as the primary point of contact for assigned members.
  • Conduct member outreach, follow-up, transitions of care, and resource referrals.
  • Complete health screenings, comprehensive assessments, and individualized care plans.
  • Monitor member progress and update care plans as needed.
  • Coordinate healthcare services and community-based resources.
  • Educate members regarding available care management programs, benefits, disease management programs, and community services.
  • Promote member engagement and adherence to person-centered care plans.

Clinical & Administrative Functions

  • Conduct telephonic and face-to-face member assessments, including home and facility visits.
  • Document all member interactions and care management activities accurately and timely.
  • Generate and distribute member correspondence as needed.
  • Maintain productivity and documentation standards.
  • Support interdisciplinary care team meetings and collaborative care planning.
  • Research and respond to member and provider inquiries.
  • Assist with audit readiness and compliance initiatives.
  • Participate in departmental projects, meetings, and training activities.

Compliance & Professional Standards

  • Practice within the scope of licensure and applicable state regulations.
  • Maintain compliance with HIPAA, NCQA, URAC, and organizational policies.
  • Protect confidential member and company information.
  • Support organizational health initiatives and business objectives.

Required Qualifications

  • Current, unrestricted Illinois license in one of the following:
    • Registered Nurse (RN)
    • Licensed Clinical Professional Counselor (LCPC)
    • Licensed Clinical Social Worker (LCSW)
    • Licensed Marriage and Family Therapist (LMFT)
  • Minimum 2 years of direct clinical experience.
  • Minimum 3 years of managed care, wellness, case management, care coordination, or population health experience.
  • Experience conducting health assessments and care planning.
  • Knowledge of medical management and health education programs.
  • Strong communication and member engagement skills.
  • Experience analyzing medical and healthcare-related information.
  • Proficiency with Microsoft Office (Word, Excel, PowerPoint) and web-based applications.
  • Ability and willingness to travel throughout the assigned territory.

Preferred Qualifications

  • 3+ years of clinical experience.
  • Government Programs experience (Medicaid, Medicare, DSNP).
  • Condition Management or Disease Management experience.
  • Complex or Catastrophic Case Management experience.
  • Patient education experience.
  • Bilingual English/Spanish.
  • Case Management Certification (CCM) or other nationally recognized healthcare certifications.
