We are currently recruiting for a Licensed Social Worker or Licensed Professional Counselor in our Behavioral Health Department. This position provides assessment, treatment, and progress monitoring for individuals with a mental health diagnosis and/or disabilities. Services are provided both in person as well as through telehealth.  

Hours of work are flexible, can be full or part-time.

Essential Duties and Responsibilities
  • Prepares for and conducts individual, group and family therapy for assigned caseload according to the treatment frequency stated on the Individual Service Plan (ISP).
  • Provides CPST and TBS services and activities as assigned.
  • Assist client in developing and implementing an individualized treatment plan.  Monitor progress in treatment and participate in treatment plan review as assigned. Modify treatment plans to comply with changes and make referrals as appropriate.
  • Complete all assigned paperwork and documents according to assigned/expected timelines, applicable standards and requirements.
  • Assist client in making discharge and/or transition plans.  Make appropriate referrals to other treatment services, both internal and external
Qualified applicants will have:
  • State of Ohio License (LSW, LISW, LPC, or LPCC);
  • Solid behavioral health assessment and clinical skills;
  • Excellent record keeping skills.
Environmental Demands
  • The work is performed in an office environment or community setting such as a workshop, school, home, etc. 
  • There is some health and safety risk due to the behavior of clients in crisis.
  • Individual must be able to implement crisis intervention procedures.
