Treasure Coast Community Health (TCCH), a long-established and highly respected healthcare provider and Federally Qualified Health Center with locations throughout Indian River County, FL is seeking a Behavioral Health Coordinator to join our team.

The Behavioral Health Coordinator is responsible for supporting the mental health care provided by the mental health care providers as appropriate. Coordinates referrals to clinically indicated services for all providers. In addition, the coordinator collaborates, initiates required documents and administrative paperwork, and ensures forms are ready in a timely manner.

Key Responsibilities:

• Coordinates referrals to clinically indicated services outside the primary care clinic (e.g., mental health specialty care, substance abuse treatment) and inside the primary clinic (e.g., social services, outreach services).

• Supports and closely coordinates mental health care with the patient's primary care provider and other treating mental health providers.

• Coordinates patients' schedules with Psychiatrists, LCSWs, LMHCs, and group therapy.

• Follows up with clients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications and reports to appropriate providers the clients' concerns/issues. Gathers all pertinent information for BH providers and relays information as needed.

• Facilitates patient engagement and follow-up in mental health care.

• Prepares patients for Psychiatrist, LCSW, and LMHC visits by updating electronic health records, greeting patients, clarifying chief complaints, current medications, and scheduling appointments.

• "Huddles" with providers to see if there are special circumstances to review (i.e., insurance, forms needed, or sensitive client issues, including if there is one where insurance does not cover the visit; ensure patients are not sent away without being seen if they are an already recognized high-risk patient).

• Schedules all provider meetings and ensures timely notifications to providers; schedules new clients at 90-minute intervals and all others at 60-minute intervals, as applicable; allows provider schedules to accommodate "admin" time.

• Schedules CFARS/FARS at varied intervals to accommodate provider schedules; coordinates with front desk reception regarding patient insurance, scheduling, and any conflicts or discrepancies.

• Assists with correspondence (letters for school/work, letters for court, school, SSI, and other entities).

• Completes phone intakes/referrals to providers; coordinates referrals from providers; assists providers with administrative tasks as necessary: inputs Cross-cut and other testing data, and start-up paperwork into EHR.

• Develops a system to track treatment plans for each BH provider and informs them when treatment plans are due to be staffed; schedules times for providers to meet for treatment plan discussion/signatures.

• Receives instructions on next visit, education, medication, keeping appointments, etc., and explains practitioner's information to the patient.

• Cross-covers Clinical Care Coordinator duties as well as other facility areas as instructed by supervisor.

• Answers phones, photocopies, faxes, and performs other clerical duties as assigned. Makes telephone contact with patients and documents all patient encounters.

• Collects co-pays and explains financial policy to patients.

Education and Experience:High school diploma or GED required. Three to six months prior reception or scheduling in healthcare and computer experience strongly preferred.
