Description

The Behavioral Health Counselor is a critical part of the primary care team in our Health Care for the Homeless Program. The Behavioral Health Counselor assesses patient needs, develops treatment plans, provides outpatient individual and group therapy, monitors progress in treatment, provides psychoeducation, and collaborates with medical and dental departments.

Responsibilities: • Actively participate as a member of the integrated primary care and behavioral health team. • Provide behavioral health screening and assessments to patients experiencing homelessness at assigned NeighborHub Health Primary Care Centers. • Ability to conduct biopsychosocial assessments and monitor ongoing treatment planning needs. • Provide counseling and brief intervention activities for behavioral health and substance abuse disorders. • Participate in Quality Improvement activities. • Work alongside physicians and offer behavioral health advice/intervention when needed. • Make necessary referrals and discuss cases with clinicians. • Demonstrate ability to document within EMR system. • Make referrals to ongoing sources of care for identified problems. • Other responsibilities as assigned.

Knowledge and Abilities Required: • Ability to work effectively with teams and a diverse array of professionals and agencies. • Knowledgeable about the region's mental health and substance use treatment system and resources. • Cultural sensitivity and willingness to work with patients experiencing homelessness and patients with HIV. • Commitment to the mission of the NeighborHub Health

Position is eligible for the National Loan Service Corp Loan Repayment Program

Position Requirements:

Education: Master's in Social Work or Clinical Mental Health Counseling from a CACREP accredited university

Professional License Requirements: Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC) in the State of Ohio

Job Type: Full-time

Pay: $55,000.00 - $65,000.00 per year
