Behavioral Health Navigator
Location: Alameda, CA (On-site)
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Type: Temporary (40 hrs/week) | W2
Pay Rate: $43.51/hr
Overview
We’re seeking a Behavioral Health Navigator to support the Enhanced Care Management (ECM) clinical team by assisting with outreach, care coordination, member/provider support, and navigation of services and resources. This role maintains an ongoing caseload with clinical-team support as needed and serves as a key liaison to assigned ECM provider partners.
Job Responsibilities
Support clinical staff with outreach, orientation, and comprehensive assessments for members who may benefit from ECM services.
Build and maintain relationships with members, caregivers, and providers; coordinate communication and follow-up based on ongoing needs.
Provide telephonic, email, and face-to-face support to help members meet care plan goals (in coordination with case managers as appropriate).
Educate members/providers on benefits, eligibility, services, referrals, and appropriate utilization.
Identify barriers and connect members to community resources (medical and social supports); confirm follow-through and coordinate additional needs.
Serve as the main point of contact for assigned ECM provider partners; assist with navigation of internal resources and supportive services.
Assist with ECM-related workflows such as referrals/authorizations, assigning members to ECM providers, and supporting oversight/monitoring activities (reports, workflows, audits).
Maintain accurate documentation and data integrity within systems; participate in meetings and assist with reporting/administrative tasks.

Qualifications
Education: Bachelor’s degree preferred (or equivalent healthcare-related professional experience).
TB test: Must have a cleared TB test prior to hire or within seven (7) days of hire.
Experience (preferred):3+ years in healthcare or healthcare-related customer service (health plan experience and knowledge of medical/insurance terminology preferred)
1+ year in care delivery/coordination (outpatient, office, home care, or inpatient), including care plan development/care coordination/discharge planning
