Position: Behavioral Health Referral Coordinator

Department: Behavioral Health

Reports to: Chief Behavioral Health Officer

FLSA Status: Non-Exempt, Hourly

Employment Status: Full-Time

Work Schedule: Monday-Friday, during clinic hours

Work Setting: In-Person, ability to commute to 93110 required

Location: Santa Barbara, California

Pay Range: $28.00-$32.00 per hour. Please note that this is the current salary range for the position. Starting pay rates are determined based on years of experience and internal equity.

WHAT WOULD I BE DOING AT THIS JOB?

American Indian Health & Services is looking for a Behavioral Health Referral Coordinator to join our Behavioral Health and Cultural Services team. This position supports patients by overseeing eligibility, coordinating appointments and reminders, managing referral caseloads, processing internal referrals, and assisting with other coordination activities that support whole-person care.

ESSENTIAL FUNCTIONS

• Tracking internal behavioral health referrals using established digital processes.
• Checking telephone messages throughout the day and processing requests in a timely manner.
• Maintaining complete and accurate patient registration information, including same-day entry of demographic updates into the electronic health record.
• Verifying and updating patient telephone numbers, addresses, and other demographic information at each encounter.
• Maintaining accurate documentation of information received, and actions taken.
• Verifying patient eligibility for Behavioral Health appointments in advance and responding promptly to identified changes to minimize interruptions to treatment.
• Ensuring that required registration documents, notices, and consent forms are completed, signed, and provided to Medical Records for placement in the electronic chart.
• Managing daily, weekly, and monthly Behavioral Health provider schedules.
• Answering scheduling-related calls and processing provider scheduling requests.
• Working with patients and staff to coordinate new Behavioral Health appointment intakes.
• Assisting patients and families with potential barriers involving the healthcare system, finances, transportation, interpretation services, or other social needs.
• Serving as a system navigator and point of contact for patients and families receiving services through Behavioral Health and Cultural Services.
• Helping patients and families obtain timely approval for necessary services or supplies.
• Identifying and using appropriate cultural and community resources.
• Establishing and maintaining professional relationships with community resources and service providers.
• Performing other coordination duties in support of whole-person care as assigned.

QUALIFICATIONS, KNOWLEDGE, AND SKILLS

• Effective verbal and written communication skills.
• Strong customer service skills and a commitment to providing helpful, respectful service.
• Strong organizational skills and the ability to manage competing priorities.
• Computer proficiency, including experience using an electronic health record.
• Ability to take initiative and follow through with delegated responsibilities.
• Dependability and accountability.
• Ability to communicate with patients clearly, concisely, and helpfully, both in person and over the telephone.
• Ability to interpret and apply standard regulations and procedures.
• Ability to maintain accurate records and files.
• Ability to operate the computer systems and software necessary to perform the position's responsibilities.
• Bilingual proficiency in English and Spanish preferred
• Experience in an Indian healthcare or community healthcare setting preferred.

PHYSICAL AND WORK ENVIRONMENT REQUIREMENTS

While performing the duties of this position, the employee is regularly required to use their hands, reach with their hands and arms, and communicate verbally. The employee is frequently required to stand, walk, and sit and may occasionally be required to stoop or crouch. The employee must occasionally lift or move up to 25 pounds. The position also requires close vision, color vision, depth perception, and the ability to adjust focus.

The employee may have potential exposure to infectious substances, hazardous chemicals, hazardous waste, and moving equipment. These risks are minimized through compliance with established safety policies and procedures. The noise level in the work environment is generally moderate.

Clinic employees may be required to remain at or report to work during public health incidents or emergencies. This may require working a non-traditional schedule or performing duties outside the employee's normal assignment during an incident or emergency.
