Overall Summary
Under the general direction of the Behavioral Health and Social Services Director, leads the day-to-day operations of the behavioral health care coordinators within the Behavioral Health Department. Provides support to BH care coordinator staff with patient and work Issues; coordinates break and lunch coverage throughout the day and time-off requests. Monitors and ensures the quality of coordinators' work by engaging in regular performance evaluations and creating performance improvement plans, as needed. Works with BH care coordinators to ensure referrals are managed/updated, reminder contacts are completed, documentation is accurate and timely, and completes warm connection requests from PCP and other staff. Develops and updates BH care coordinator workflow and policy and procedures as needed. Ensures BH care coordinators are participating in regular check-ins to prepare for dally operations. Monitors staff for completion of duties, assists with scheduling, training and oversight as directed by the BH Manager. In support of patient care at Bay Area Community Health (BACH), provides a variety of clinical and support duties and language Interpretation as needed.
