Responsibilities

• Monitor and document patient behavioral health status, including mood and safety indicators.

• Maintain accurate, timely documentation in electronic health records and state systems.

• Monitor and manage escalation procedures for risk of harm, including safety checks.

• Assist clinicians with intake, assessments, and care plan updates during shifts.

• Implement behavioral health interventions under supervision, following approved protocols.

• Collaborate with multidisciplinary team to coordinate care and ensure continuity.

• Meet performance targets and contribute to quarterly pipelines for patient outcomes and service delivery.

• Maintain clean, compliant clinical environment and adhere to confidentiality policies.

• Engage in ongoing professional development and participate in training and certifications.

• Support patient engagement initiatives to improve adherence and attendance.

• Capture, track, and report key performance indicators related to caseload and outcomes.

• Act as a role model for growth-minded behavior and contribute to a merit-based culture.

Experience/Qualifications

• Ability to assess risk and execute escalation and safety procedures.

• Demonstrated ability to monitor and document patient behavioral health status.

• Proficiency in electronic health records and state system documentation.

• Implement behavioral health interventions per approved protocols.

• Track and report key performance indicators and caseload metrics.

• Assist with intake, assessments, and care plan updates under supervision.
