Faith-based family health care center seeking a full-time (40 hours per week) Behavioral Health Therapist to join our growing behavioral health team! The Behavioral Health Therapist acts as an advocate and helps clients gain access to community-based resources and counsels patients on a 1:1 basis regarding their personal issues, such as mental illness, trauma, addiction and abuse.

QUALIFICATIONS
• Must be an LCSW or LPC • At least 3 years of experience post licensure • Experience working with patients who have suffered from trauma, abuse and chemical dependency • Experience working with the healthcare needs of underserved populations • Experience in a health care setting helpful • Bilingual/Spanish speaking a plus
KEY JOB RESPONSIBILITIES
• Provides 1:1 psychosocial counseling for ELFHCC patients • Complete a psychosocial assessment that includes social, economic, cultural, age-related factors, history of child, elder or domestic abuse, prior psychiatric or substance abuse treatment, family and social support systems • Coordinates and plans programs and activities to meet the patients social and emotional needs • Collaborates with other members of ELFHCCs behavioral health team to provide co-management activities for patients who require additional services • Develops and implements a comprehensive, interdisciplinary plan of care which includes patient preferences • Establishes a plan for regular contact with each patient to review progress and assess the potential for additional needs
