Compensation: Approximately $22.00 - $27.00 per hour. All individual pay rates are calculated based on the candidates experience and internal equity.

Overview of Role:

Functions as a liaison and advocate for patients between their care team, specialty care, and/or other relevant staff and assists patients in navigating referrals to community resources and specialty care.

Job Profile:

  • Do you have your Bachelor in Psych, Social Work or Sociology and looking to get experience in mental health?
  • Are you passionate about helping people continue to engage with services dealing with trauma, anxiety and depression?
  • Do you have experience with the criminal justice system?
  • Do you want to develop some case management skills?

  • Is building relationships with clients easy for you?
  • You will report to the Program Supervisor.
  • This is an onsite position.

What we need for this job:

  • Bachelors in Psychology, Social Work, Counseling or related degree.

  • 5 years of experience can be substituted for a degree.

  • Collaborative, and client centered.

  • Organization is necessary to managing client information, appointments and follow up items.

  • Collaborative team member who is ready to jump in, support our clients and team.

  • A 40 hrs. per week role and with benefits (medical, dental, vision, life, disability and retirement plan).

Knowledge, Skills and Abilities:

  • Bilingual in Spanish required.
  • Sensitivity to under resourced individuals and communities.
  • Case management for complex patients.
