Responsibilities:
- Assist residents or clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support.
- Monitor patient health by observing vital signs and reporting any irregularities to healthcare professionals promptly.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary.
- Maintain a clean and safe environment by performing light housekeeping duties, including cleaning and sanitation tasks.
- Support medication administration following HIPAA regulations and care plans established by healthcare providers.
- Provide emotional support and companionship to promote mental well-being and social engagement.
- Utilize specialized equipment such as Hoyer lifts or resident's beds to ensure safe patient transfers and mobility assistance.
Skills:
- Proven experience in home health, hospice care, assisted living, or long-term care environments.
- Knowledge of patient monitoring techniques, vital signs measurement, and medication administration.
- Familiarity with dementia care, Alzheimer's care, memory care, and working with individuals with developmental disabilities or autism.
- Ability to perform heavy lifting safely while maintaining proper body mechanics.
- Strong understanding of HIPAA regulations to ensure patient confidentiality and privacy.
- Excellent communication skills for effective interaction with residents, families, and healthcare teams.
- Experience with care planning, assisted living procedures, and social work practices is highly desirable.
