A-Line Staffing is now hiring a Bilingual Case Management Coordinator (Spanish/English) in the Hialeah area. This position offers competitive pay and the opportunity to support Medicaid Long Term Care members through a combination of remote coordination and field-based care management.


Bilingual Case Management Coordinator Compensation

Pay Rate: Up to $34.13/hour


Schedule

Monday – Friday, 8:00 AM – 5:00 PM


Additional Information

  • Remote position with field travel (approximately 50–75%)
  • Candidates must reside in Miami-Dade County, FL, specifically in Hialeah (ZIP codes: 33010, 33012, 33013) and surrounding areas
  • Fluent Spanish/English required (must be able to read, write, and speak fluently)
  • Patient-facing role visiting members in homes, assisted living facilities, and skilled nursing facilities
  • Training conducted remotely via Microsoft Teams (approximately 1–2 weeks)
  • Mileage reimbursement available
  • This is not a nursing role — nurse candidates will not be considered
  • Opportunity to manage a structured caseload supporting long-term care members
  • Must be able to multitask and adapt in a fast-paced environment

Bilingual Case Management Coordinator Responsibilities

  • Coordinate case management activities for Medicaid Long Term Care/Comprehensive Program members
  • Conduct telephonic and face-to-face member assessments
  • Develop, implement, and monitor individualized care plans
  • Collaborate with providers, primary care physicians, and interdisciplinary teams
  • Facilitate prior authorizations and coordinate healthcare services
  • Provide education related to condition management and medication adherence
  • Connect members with community-based resources and support services
  • Monitor care plan progress and identify barriers to care
  • Escalate quality-of-care concerns through established channels
  • Utilize motivational interviewing techniques to encourage member engagement
  • Maintain accurate documentation in compliance with regulatory guidelines
  • Support members in making informed healthcare decisions

Bilingual Case Management Coordinator Requirements

  • Fluent in Spanish and English (Required — must be conversational in both languages)
  • Must reside in Miami-Dade County, FL, preferably in Hialeah ZIP codes 33010, 33012, or 33013
  • Bachelor’s degree required (Social Work or related field required — No Nursing degrees)
  • Minimum 1+ year of experience in behavioral health or long-term care setting required
  • Ability to travel approximately 50–75% locally to meet members
  • Strong organizational and multitasking skills
  • Proficiency with Microsoft Word, Excel, and Outlook
  • Strong verbal and written communication skills
  • Verifiable High School Diploma or GED required

  • Identifying appropriate patients for care management
  • Coordinate the overall interdisciplinary care plans for home health/hospice patients from admission to discharge
  • Evaluate outcomes of care with the interdisciplinary team and medical case managers
  • Providing patient case management services to assigned populations
  • Performing ongoing telephonic case management and treatment planning
