About the Role:

Wilder Recovery Services is seeking a compassionate Bilingual Karen Treatment Coordinator to provide client-centered treatment coordination and support for individuals receiving substance use and co-occurring mental health services. This position plays a key role in ensuring clients have timely access to services, reducing barriers to care, and supporting continuity throughout the treatment process.

Primary Responsibilities include manage referrals and coordinate access to treatment services, answer incoming calls and respond to inquiries, complete client intakes, provide resources, education, and ongoing support to clients and their families, schedule appointments, transportation, and other services that support engagement in treatment. Responsibilities also include assisting with continuity of care by coordinating with family members, caregivers, and other significant supports, when appropriate, to promote client engagement in treatment planning; coordinating and following up on medical services identified in the client's treatment plan, providing resources, facilitating referrals to substance use disorder treatment, mental health services, primary care, and other services as identified through the comprehensive assessment or treatment plan; assisting clients in accessing community resources, and accurate documentation of all treatment coordination activities in accordance with agency policies and regulatory.

Job Qualifications

This position requires strong organizational skills, cultural responsiveness, and the ability to build trusting relationships with clients, families, healthcare providers, and community partners. Must be fluent in Karen and English and is passionate about improving access to culturally responsive behavioral health services. Is skilled in the process of identifying and assessing a wide range of client needs and is knowledgeable about local community resources and how to use those resources for the benefit of the client. Has successfully completed 15 hours of education or training on substance use disorder, co-occurring conditions, and care coordination for individuals with substance use disorder or co-occurring conditions that is consistent with national evidence-based standards (Training will be provided if candidate does not have this currently).

Must meets one of the following criteria:

(i) Has a bachelor's degree in one of the behavioral sciences or related fields;

(II) Has a high school diploma or equivalent; or

(iii) Meets the qualifications as a mental health practitioner or alcohol and drug counselor

At least 1,000 hours of supervised experience working with individuals with substance use disorder or co-occurring conditions preferred.
