Bilingual Long-Term Care Service Care Manager. This is a

Pay Range: $25 - $29/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:- Manage a caseload of 50-70 members, focusing on geriatric long-term care.
- Conduct assessments with members, caregivers, or providers to develop care plans.
- Monitor service delivery and follow up with members through in-person visits and telephonic contact.
- Authorize and coordinate referrals for services, ensuring no gaps in care.
- Collaborate with discharge planners, physicians, and other stakeholders for effective care coordination.
- Assist members with filing and resolving complaints and appeals.
- Maintain compliance with new contract requirements effective 2/1/25.
Key Requirements and Technology Experience:- Key Skills; Experience with discharge planning.
- Familiarity with TruCare software.
- 2+ years of care management experience, with a strong emphasis on field experience.
- Bachelor's degree in a relevant field (e.g., Psychology, Sociology, etc.).
- Valid Driver’s License.
- Experience managing high caseloads in a fast-paced environment.
- Proficiency with electronic medical health records.
- Bilingual in Spanish is required.
