Salary and Benefits
Base Salary: $105,000 – $135,000 (commensurate with experience and region)
Incentive Bonus: $15,000 – $25,000 annually based on quality outcomes
Physician Partnership Track available
6 Weeks of Paid Time Off (inclusive of vacation, holidays, and CME)
Annual CME Allowance: $5,000

Responsibilities
Full-Time Schedule: Monday to Friday, 8:00 AM – 5:00 PM
Patient Load: 13–16 patients per day
Appointment Duration:
Routine Visit: 20 minutes
New/Complex Visit: 40 minutes
Preventive, Value-Based Care Model
Support Team Includes:
Medical Assistant
Scribe
Social Worker
Onsite Laboratory and Phlebotomy
Specialist Access via RubiconMD
EMRs: Canopy and Greenway
Participation in Community Health Events and Wellness Programs
Patient Transportation Within a 5-Mile Radius
Collaborative Setting with 3–5 Providers Per Clinic
Requirements
Master of Science in Nursing, Doctor of Nursing Practice, or equivalent accredited NP program
Active Illinois NP License and DEA License
National Board Certification in:
Family Nurse Practitioner (FNP)
Adult-Gerontology Primary Care NP (AGPCNP)
Adult NP (ANP)
Gerontological NP (GNP)
Bilingual Spanish Required
Experience with HCC Documentation, ICD-10 Coding, and Health Risk Assessments (Preferred)
