- Base salary range: $125,000 – $130,000+ (flexible depending on experience)

Responsibilities
- Monday – Friday 8AM – 5PM
- No nights or weekends
- Minimal call responsibility (1 week, 3–4 times per year; triaged through call center)
- Deliver comprehensive, value-based primary care to a geriatric patient population
- Manage small patient panels (12–16 patients/day) in an outpatient setting
- Conduct in-clinic care with occasional home visits and telehealth appointments
- Perform physical exams, assessments, and diagnostic testing
- Create and implement individualized care plans focused on chronic disease management
- Educate patients and families on medication, nutrition, prevention, and disease progression
- Collaborate with a multidisciplinary team including MA, social worker, pharmacist, and specialists
- Document care accurately to support risk adjustment and quality metrics
- Prescribe medications within state scope of practice
Requirements
- Master's degree in Nursing
- Board certification as a Nurse Practitioner (Adult, Adult-Gero, or Family)
- Minimum of 2 years outpatient primary care experience
- Autonomous practice capability required
- Experience with value-based care preferred
- Strong background in geriatrics and chronic disease management
- Proficiency with EMRs (Athena preferred)
- Must hold an active NP license in the state of California and DEA certification (or be eligible)
- Spanish speaking required
- Experience in wound care, ABIs, injections, or imaging interpretation preferred
