About the Role

This full time position is primarily outpatient focused,with approximately 70% of your time spent in clinic and the remaindersupporting dialysis and occasional inpatient rounding. You will partner closelywith nephrologists and interdisciplinary care teams to evaluate, diagnose, andmanage patients with chronic kidney disease, hypertension, dialysis needs, andrelated medical conditions.

Responsibilities include:

• Conducting patient assessments and physical examinations
• Developing and implementing treatment plans
• Managing medications and chronic conditions
• Educating patients and families on disease management
• Collaborating with physicians and care teams to deliver exceptional patient outcomes
• Maintaining accurate and timely medical documentation

Requirements

Applicants should have:

• Licensed or license eligible Nurse Practitioner or Physician Assistant inCalifornia

Candidates with nephrology, dialysis, oncology, hospitalmedicine, or other specialty care experience are highly encouraged to apply.Providers with prior dialysis nursing experience will also be stronglyconsidered. Motivated new graduates are welcome.
