Help Members Take Charge of Their Health

Alignment Health is seeking a compassionate and motivated Bilingual Spanish Health Coach & Educator to support members through in-home, telehealth, and telephonic visits across Los Angeles and Orange County.

In this rewarding role, you'll build meaningful relationships with members, caregivers, and families while providing health coaching, education, encouragement, and access to resources that help improve overall health and well-being. *Candidates must be comfortable conducting home visits throughout Los Angeles and Orange County, California.

What You'll Do

• Conduct home and virtual visits with Alignment Health members.
• Provide wellness coaching to help reduce high-risk behaviors and support healthy lifestyle changes.
• Collaborate with Advanced Practice Clinicians and Social Workers to create personalized action plans.
• Connect members with community resources and social services.
• Deliver health education and wellness materials to members, caregivers, and families.
• Coordinate care with internal teams and community partners.
• Support member satisfaction, engagement, and retention through exceptional service.

Minimum Requirements:

Experience:

• Minimum 1+ year of experience as a wellness, health coach or educator
• Spanish/English Bilingual
• Knowledge of health, wellness, nutrition, exercise, and behavior change principles
• Excellent interpersonal, communication, and relationship-building skills.
• Ability to work independently and remotely.
• Strong organizational, problem-solving, and time-management skills.
• Proficiency in Microsoft Excel, Word, and PowerPoint.
• Ability to maintain confidentiality and comply with HIPAA requirements.
• Ability to work a flexible schedule.

Education:

• High School Diploma or GED. Associate's degree or two years additional experience in lieu of education.

Preferred:

• Senior care experience.
• Bachelor's degree.
• Medical Assistant License

Pay Range: $48,373.00 - $72,559.00
Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.
