We are growing our home care team in Brooklyn and are looking for a Bilingual Patient Care Coordinator who is fluent in Spanish and English. This position is ideal for someone who enjoys working directly with patients and families, coordinating services, and making sure individuals receive the support they need to remain safely and comfortably at home.

Your Role

As a Patient Care Coordinator, you will help ensure that patients receive timely, organized, and compassionate home care services. You will work closely with patients, families, caregivers, and healthcare professionals to coordinate care and address patient needs.

Your responsibilities will include:

• Build positive relationships with patients and their families and serve as a reliable point of contact
• Coordinate home care services according to each patient's individual needs and care plan
• Assist with scheduling, follow-ups, and in-home assessments
• Communicate care plans, services, and expectations clearly to patients and caregivers
• Follow up on patient needs and changes in their care situation
• Maintain accurate and timely patient records and documentation
• Communicate and coordinate with healthcare providers, home health aides, and other members of the care team
• Help ensure services are delivered in accordance with agency policies, regulatory requirements, and quality standards
• Respond promptly and professionally to patient and family concerns
• Support patients throughout their care journey with empathy and respect

What You'll Bring

• At least 1 year of experience in care coordination, case management, patient services, clinical support, or a related healthcare setting
• Home care experience preferred
• Fluent in Spanish and English required
• Strong communication and interpersonal skills
• Excellent organizational skills and attention to detail
• Proficiency with Microsoft Office and electronic health record (EHR) systems
• Ability to manage multiple priorities in a fast-paced environment
• Professional, compassionate, and patient-focused approach
• Ability to work effectively both independently and as part of a team
