Bilingual Wrap-Around Services Coordinator (Social Worker License Required)

1LT Charles W. Whitcomb Middle School
Organizational Relationship or Line of Authority:
Works under the supervision of the Executive Director of Clinical Services.
Salary:
Annualized salary of $78,000
This position is a grant funded position through the McKinney-Vento (MKV) Grant.
Schedule:
Full-time, 12-month position, 218 days
8 hours per day, Monday through Friday (8am-4pm) with flexibility to shift schedule as necessary for evening work with parents
Location:
Registration and Parent Outreach Center, Marlborough Public Schools
Statement of Duties:
Serve as the Marlborough Public Schools (MPS) point of contact/liaison with outside agencies & organizations for supports, wrap-around services for students, including but not limited to, counseling, food bank, South Middlesex Opportunity Council (SMOC), Marlborough Hospital)
Support enrolled students & families, including those who are newly enrolling in the district
Primary resource contact for MKV and other families that demonstrate a need to provide support with food, shelter and health care resources
Establish a database of existing resources within the MPS community, and create, update and maintain a working resource (document) for staff to readily access with wrap-around services available to students and families.
Serve as a facilitator to school administrators and School Adjustment Counselors (SACs) on availability of these wrap around services.
In collaboration with RPOC nurse and translators, assist families that require support with:
Completion & submission of MassHealth applications
Obtaining primary care physician, if applicable
Obtaining necessary immunizations & physicals
Applications for SNAP, Transitional Housing Assistance, etc.
Referrals to MetroWest Legal
Consult with SACs regarding complex student & family issues-facilitates referrals to wrap-around services, as needed
