This role focuses on providing culturally responsive, trauma-informed care coordination, outreach, engagement, and resource navigation to support maternal health and family stability. The ideal candidate is passionate about serving underserved communities, skilled in relationship-building, and committed to whole-person care.
Responsibilities:

  • Support members in addressing social drivers of health, including housing, food access, transportation, and healthcare navigation.
  • Collaborate with healthcare providers, community organizations, and interdisciplinary teams to support referrals and member care coordination.
  • Maintain ongoing communication and follow-up to support successful member engagement and retention.
  • Maintain timely and accurate documentation in accordance with ECM and organizational standards.
  • Participate in trainings, supervision, outreach activities, and team meetings.
Preferred Qualifications:
  • Bachelor’s degree in social work, psychology, public health, human services, or related field preferred.
  • Experience in ECM, CalAIM, care coordination, outreach, case management, maternal health, behavioral health, or community-based services preferred.
  • Experience working with Medi-Cal populations and underserved communities.
  • Knowledge of social drivers of health and community resource navigation.
  • Strong communication, engagement, and relationship-building skills.
  • Bilingual English/Spanish preferred.
  • Experience working with pregnant/postpartum populations, child welfare systems, or family support services is a plus.
What We’re Looking For:
We are looking for individuals who are compassionate, dependable, and passionate about serving underserved communities. The ideal candidate values relationship-building, whole-person care, and culturally responsive engagement and is committed to supporting pregnant and postpartum members with empathy, professionalism, and consistency.
Successful candidates are proactive communicators, comfortable with outreach and community-based work, and able to build trust with individuals navigating complex healthcare and social systems.

What We Offer:
  • Opportunity to support a growing Birth Equity initiative.
  • Mission-driven and supportive work environment.
  • Hybrid and community-based work opportunities.
