    Department Overview

    • Inpatient Medicine: Assist with psychosocial assessments and connecting patients to resources based on social needs
    • Psychiatry: Lead patient groups and manage acute psychiatric stabilization across inpatient and crisis units, as well as coordinating discharge planning.
    • Emergency Department (ED): Deliver rapid crisis intervention, safety planning, and trauma support in our Level 1 ED.
    • Inpatient Transplant: Conduct long-term pre- and post-op evaluations and coordinate highly specialized medical placements.
    • Outpatient Transplant: Provide continuous longitudinal tracking, compliance profiling, and resource matching for candidates.
    • Women and Infants: Complete psychosocial assessments and facilitate resource connections for both mother and baby.
    • Outpatient Clinics: Manage preventative care triage and build long-term relationships with diverse clinic populations.
    • AWARE Program: Provide immediate crisis support, safety planning, and emergency funds for Domestic/Intimate Partner Violence survivors.

    Overview

    Barnes-Jewish Hospital at Washington University Medical Center is the largest hospital in Missouri and is ranked as one of the nation's top hospitals by U.S. News & World Report. Barnes-Jewish Hospital's staff is composed of full-time academic faculty and community physicians of Washington University School of Medicine, supported by a house staff of residents, interns, fellows and other medical professionals. Recognizing its excellence in nursing care, Barnes-Jewish Hospital was the first adult hospital in Missouri to be certified as a Magnet Hospital by the American Nurses Credentialing Center.

    The CM Services reports through the division of Patient Care Services lead by the Chief Nursing Executive, Vice President of Patient Care Services. The CM Service is comprised of three departments: Case Management, Utilization Management, and Social Work. Through the Triad Model of Care Delivery, the objectives of the CM Services are for all patients to have access to healthcare through the BJH continuum and appropriate, safe discharge plans. The SW department is open 24 hours/day while the CM department is open 20 hours/day. Both departments provide coverage throughout the hospital including the Emergency Department, Outpatient Clinics, Registration sites, and Health Information Management. The Service employs registered nurses, social workers, a license practical nurse, a customer service representative, and administrative assistants.

    Role Purpose

    Under direction of a physician, assists patients and their families in their residences or outpatient/inpatient settings to adapt and plan an interdisciplinary approach to treating the patient-family system.Employees working at an Illinois facility are required to have an Illinois state social work license or be able to take and pass the Illinois state social work licensing exam within 90 days of the first day of employment.

    Responsibilities

    Provides direct clinical services to patients and families.Plans and implements social work services on assigned clinical service or unit in collaboration with the healthcare team.Collaborates with community human service agencies to meet the needs of patients, families, and the hospital.
