Job DescriptionSummary
The speech-language pathologist is responsible for implementationof standards of care for speech-language pathology services andfor adherence to all conditions in the Service Agreement.
Essential JobFunctions/Responsibilities
Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.
Identifies functional speechdefects and establishes a planof care toimprove patient’s function.
Administers speech therapyprogram utilizing specialized therapeutic technique and/or equipment.
Recommends communicationdevices/aids as indicated.
Confers with referring physician andother organizationpersonnel to ensure coordinated and comprehensive care contributes to the totalplanof care.
As appropriate, participates in case conferences, in-services andother meetings required to ensure coordinated and comprehensive care.
Prepares and submits notes per organizationpolicy.
Maintains clinical competencyin speech-language pathologypractice and theory.
Identifies patient andfamily/caregiver needs for other services and refers as appropriate.
Instructs patient, family/caregiver andother Organizationhealth care personnelin the patient’s treatment regime as indicated.
May supervise home health aide inprocedures directly related to the speech-language therapyplan of care.
Performs other duties as required tofacilitate the deliveryof speech-language pathology services.

Position Qualifications
Possesses a master’s or doctoral degree in speech-language pathology (SLP), and who meets either of the following requirements:
Is licensed as a speech-language pathologist by the State in which the individual furnishes such service; or If the State does not license speech-language pathologists:
Must successfully completed 350 clock hours of supervised clinical practicum (or is in the process of accumulating such supervised clinical experience);
Performed not less than 9 months of supervised full-time SLP services after obtaining a master’s or doctoral degree in SLP or a related field; and
Successfully completed an approved national examination in SLP
A minimum of one (1) year's clinical experience in Speech-Language Pathology.
Demonstrates good verbal and written communication, and organization skills.
Possesses and maintains current CPR certification.
Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
