The Role- Provide comprehensive primary care to senior patients
- Focus on prevention, chronic disease management, and outcomes
- Work within a fully integrated care team
- Build long-term patient relationships with smaller panels
- Utilize data and support tools to improve quality metrics
Ideal Candidate- Board Certified in Family Medicine or Internal Medicine
- Passion for senior-focused or value-based care
- Strong communicator who thrives in a team setting
- Open to VBC (experience helpful, not required)
- Focused on quality, outcomes, and patient relationships
