Our client is a well-established and growing home health agency serving the Clearwater market with a patient census exceeding 500. They are seeking an experienced Director of Nursing to provide clinical leadership, operational oversight, and mentorship to a high-performing home health team.

This is a hands-on leadership role for an experienced Registered Nurse (RN) or Physical Therapist (PT) with a strong background in Medicare-certified home health. The Director of Nursing will oversee clinical operations, ensure regulatory compliance, support quality initiatives, and partner closely with agency leadership to drive exceptional patient outcomes.

The ideal candidate is an organized, collaborative leader who thrives in a fast-paced environment, understands the complexities of managing a large census, and is passionate about developing clinical teams while maintaining the highest standards of patient care.

Primary Responsibilities

  • Provide day-to-day clinical leadership for a high-volume home health agency

  • Oversee clinical staff including nurses, therapists, and other interdisciplinary team members

  • Ensure compliance with Medicare Conditions of Participation and all federal, state, and agency regulations

  • Review clinical documentation for accuracy, quality, and regulatory compliance

  • Provide guidance and oversight regarding OASIS-E documentation and clinical best practices

  • Monitor quality metrics, patient outcomes, and performance improvement initiatives

  • Partner with scheduling, intake, and operations teams to ensure efficient delivery of care

  • Mentor, coach, and develop clinical staff to promote excellence and accountability

  • Participate in case conferences, care planning, and interdisciplinary team meetings

  • Assist with survey readiness, audits, and accreditation activities

  • Support recruitment, onboarding, and retention of clinical staff

  • Collaborate with executive leadership to improve operational performance and patient satisfaction

Qualifications

  • Current Registered Nurse (RN) or Physical Therapist (PT) license in the State of Florida

  • Recent leadership experience within a Medicare-certified home health agency required

  • Strong knowledge of Medicare regulations and Conditions of Participation

  • Extensive experience with OASIS-E documentation and compliance

  • Experience managing clinical teams within a high-volume home health environment

  • WellSky EMR experience is highly preferred

  • Excellent leadership, communication, and organizational skills

  • Ability to analyze clinical performance metrics and implement process improvements

  • Strong critical thinking and decision-making abilities

  • Passion for delivering exceptional patient care while supporting and developing clinical staff

Compensation & Benefits

  • Base salary of $100,000 - $115,000
