Description
The Breast Health Navigator reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The Breast Health Navigator supports Patient Care Services (PCS) by overseeing the care of breast cancer patients in Northwestern Medicine's West and Northwest Regions. The Navigator works collaboratively with all members of the multi-disciplinary team to establish an evidence based continuum of care plan with the goal of optimizing clinical outcomes. The main responsibilities are patient rounding, review of documentation of required care processes, coordination or direct provision of patient education and development of a plan with case management and our community-based and outpatient services appropriate for level of complexity. With Medical Director establishes clinical standards, order sets, policies and procedures for the provision of services. The individual in this position will be responsible to identify care processes in need of improvement and proactively act to resolve unfavorable outcomes. Will serve on teams in an advisory capacity regarding updates to protocols, procedures, care guidelines and quality improvement.
