GENERAL PURPOSE:
Under the supervision of the Director of Patient Support Services, the CalAIM Clinical Consultant performs Clinical Consultation activities for CalAIM. They will determine if the Members Care Management needs are sufficient to be managed by an ECM Lead Care Manager and make recommendations to implement in the patient Care Plan.
MAIN RESPONSIBILITIES & DUTIES:
Provides Clinical oversight and ensures comprehensive intake assessments and care plans as directed by the Director of Patient Support Services.
Provides clinical advice to the following Departments:
CalAIM: RN clinical consultants provide clinical expertise and guidance to the CalAIM Program Manager and Lead Case Managers. They can help with patient intakes and care plans, allowing for the appropriate customization of individualized care plans to address specific patient needs. They can also help identify and manage high-risk patients, ensuring they receive the appropriate care and support.
Care Coordination: RN clinical consultants provide guidance to Care Coordinators on individualized patient care plans, helping them provide seamless and well-coordinated care tailored to each patient's unique needs. They can also ensure that the patient receives the appropriate level of care after being discharged from the hospital and provide guidance to decrease utilization.
Referral Center: RN clinical consultants provide clinical consulting to the LVN Referral Center Manager.
Call Center: RN clinical consultants provide clinical consulting and expertise to the call operators on phone calls and the Call Center Manager.
Transportation Manager: RN clinical consultants provide clinical consulting to the Transportation Manager as needed for patients being transported to and from appointments.
Treat all clients, staff, and the community with respect and courtesy.
Utilizes effective means to resolve conflict.
Utilize effective verbal and written communication skills.
Other duties as assigned by supervisor.
