What We Are Looking For Job Title: Call Center Registered Nurse Schedule: Saturday and Sunday 8:30am - 4:30pm, Every other weekend Salary: $500 per shift (8 hours) Job Summary: The Call Center nurse receives phone calls which they manage over the phone or they assign a staff member a visit to manage the patient's symptoms and pain; a patient death visit; or any other HHA, LPN, or RN visit they feel is needed to assist a patient or family member. The Call Center RN will consult with SW and/or SC if their care is needed by a patient and/or family. The call center RN works 8:30a-4:30p on weekend days and designated holidays. The Call Center nurse role is an expansion of the care provided by the nursing teams during the week. The role is essential to ensure 24/7 nurse availability for the hospice patient. Hybrid Position: This office-based position can be flexed to remote; decision to be made in conjunction with reporting supervisor. As warranted by the organization's needs, being present in the office can be required at any time. Essential Job Functions/Responsibilities: Present a positive image of Hospice in all contacts. Provides professional nursing care by utilizing all elements of nursing process. Assesses and prioritizes the needs of terminally ill patients and their family via telephone. Provide staff for timely visits to home, hospital or skilled nursing facility in any part of Orange/Sullivan counties anytime during assigned shift. Reviews with patient/family/caregiver medications and treatments as prescribed by the provider. Counsels the patient/family/caregiver in meeting nursing and related needs. Provides health care instructions to the patient/family/caregiver as appropriate per assessment and plan. Communicates with Hospice IDT members on a timely basis regarding changes in patient's condition that occurs during their shift. Communication: Completes, maintains and submits accurate and relevant clinical documentation regarding patient's condition and care given within 24 hours. Records pain/symptom management and changes/outcomes as appropriate. Communicates with the provider regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians orders as required. Communicates as appropriate to meet the patient/family needs with community health related persons to coordinate the care plan. Teaches the patient/family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients/families/caregivers. Works in concert with the other Hospice Interdisciplinary Team Members. Educates patient/family/caregiver re: safety and assists in providing a safe environment. Assists the patient/family/caregiver and other team members in providing continuity of care. Works in cooperation with the family/caregiver and Hospice Interdisciplinary Team Members to meet the emotional needs of the patient/family/caregiver. Additional Duties: Ensures that arrangements for equipment and other necessary items and services are available when required. Supervises ancillary personnel and delegates responsibilities when required. Responsible for supervising hospice aides, licensed practical nurses and contracted aides when required. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. Fulfills the obligation of requested case assignments. Actively participates in Performance Improvement program.
