    PRIMARY FUNCTIONS:

    The Cancer Screening Program Coordinator is an integral member of Neighborhood Health's cancer screening team. This role involves both program oversight, direct patient navigation, and data entry. The Coordinator is responsible for oversight of Neighborhood Health's Virginia Department of Health (VDH) - Every Woman's Life (EWL) program and meeting annual goals with respect to overall patient screening numbers, timely follow up, accurate data collection and data entry, and receipt of screening reports. They work closely with the Cancer Screening Program Manager to ensure that patients who are referred for breast and cervical cancer screening via the EWL program are connected to cancer screening and diagnostic services in a timely fashion and that patient data is accurately collected, entered, tracked, and analyzed for reporting to funding agencies. The Coordinator will also assist with all aspects of EWL patient navigation including entering data into Catalyst (VDH's EWL database) for reimbursement.

    EWL Program Oversight

    • Ensure high-quality patient care: Work closely with Neighborhood Health's cancer screening multi-disciplinary team - including medical providers, nurses, medical assistants, other patient navigators, and the Quality Team - to meet breast and cervical cancer screening goals.
    • Participate in periodic cancer screening team meetings to collaborate on strategies to achieve cancer screening goals.
    • Develop workflow to integrate breast cancer risk assessments for EWL patients, train EWL team members, and ensure it is being offered and correctly documented.
    • Track specialty referrals for patients at high risk of developing breast cancer.
    • Develop, maintain, and analyze operational reports related to EWL enrollment, screening and diagnostic completion and timeliness, outstanding patient follow-up activities, no-show rates, and other program performance metrics. Identify trends and opportunities for improvement and communicate findings to program leadership.
    • Assist Program Manager with responding to patient data inquiries from VDH and other partners.
    • Receive incoming referrals (self-referrals and referrals from partner agencies): Track patients for connection to care, track patient data, and provide results back to referring providers.
    • Assist with reporting data for grant-funded transportation provided to patients and track grant expenditures.
    • Mobile mammography partnerships: Coordinate logistics with partners, office managers, and clinical staff.
    • Conduct regular EWL team check-ins to assess workloads, productivity, outreach activities, and follow-up completion, and assist with workflow management and documentation review.

    • Serve as a resource to patient navigators by assisting with complex patient cases, developing strategies to address barriers, and best practices for improving patient engagement and screening completion.

    EWL Patient Navigation

    • Assess patient program eligibility: Review referred patients for EWL program eligibility.
    • Schedule mammogram appointments: Work one-on-one with patients to schedule mammogram appointments with local radiology partners; send text/voice appointment reminders; and arrange for transportation if needed. Submit billing forms to radiology partner to ensure patient is not billed at time of appointment.
    • Mobile mammography partnerships: Assist with scheduling appointments, ensure required patient information is available/accessible for mobile mammography partner, and provide onsite assistance during screening events.
    • Assess patient lifetime risk for breast cancer using standardized screening tools; record results in chart and notify medical provider.
    • Assess patient cervical cancer screening status and request appointments for patients due for screening and track for completion.
    • Ensure completion of breast and cervical cancer screening: Track appointment attendance and receipt of reports.
    • Ensure follow-up for patients when additional testing is recommended.
    • Conduct appropriate follow-up with patient if screening is not completed.
    • Chart all patient interactions in electronic medical record.
    • Ensure all patient data is accurately tracked at all stages of cancer screening process.
    • Enter data accurately into Catalyst on a regular basis.

    REQUIRED QUALIFICATIONS Skills, Competencies, Education

    • Bachelor's Degree required
    • Bilingual skills required (Spanish/English)
    • Previous experience working with vulnerable communities, particularly the uninsured
    • Experience with data management: creating, using and maintaining detailed Excel spreadsheets
    • Experience working in an EMR or database preferred (training provided)
    • Strong organizational skills, attention to detail and accuracy
    • Good interpersonal communication and customer service skills
    • Knowledge of HIPAA medical privacy requirements (training provided)
    • Familiarity with medical terms and technology (not required, but desired)
