Summary:

Cardiac Rehabilitation Specialist works with patients who have diseases of the lungs and/or heart and helps them improve or recover from their conditions; these can range from heart disease and heart attacks to respiratory problems, such as asthma. These specialists educate patients so they can care for themselves when at home, so strong communication skills are important in this position. They usually work with other medical personnel in order to understand patients' situations, and will work with them in order to understand what has brought on their conditions to determine what changes in medical treatments should be made. They may also give advice on physical activities that should be done and what sorts of dietary changes may be necessary.

Responsibilities:

  • Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
  • Patient assessment, to include current medical status and readiness to learn/mobilize, cardiovascular risk factors, medical history, contraindications to exercise, ECG monitoring/interpretation, and vital signs.
  • Documents ambulation and patient’s response/tolerance appropriately in electronic medical record (EMR).
  • Documents home program teaching, ADLs, post-discharge instructions, Phase II information, and other education discussed (smoking cessation, CHF, stress, depression, diabetes, nutrition, medications, etc.) in EMR.
  • Communicate with providers to obtain physician referrals/orders, report patient status changes/untoward events, request patient information/medical records, schedule referred patients in EMR, and create new patient charts.
  • Accurately applies telemetry transmitter to the patient with proper skin preparation and concern for patient privacy; modify electrode placement to change lead viewed on the monitor; troubleshoot problems with transmitters/monitor.
  • Generates appropriate exercise prescriptions for classes, and modifies as necessary based on signs, symptoms, and clinical indicators.
  • Completes individualized treatment plan, including initial assessment, monthly follow-up visits, and discharge outcomes measurement, to include goals, interventions, and education provided.
  • Case management responsibility for personal management of assigned patients.
  • Maintains current CPR/ACLS and stays abreast of current exercise and CR guidelines for ACSM, AACVPR, AHA, and other industry authorities; recognizes contraindications to activity and is knowledgeable of cardiac emergencies and responds appropriately.
  • Office duties to include answering phones, obtaining physician orders for wellness memberships, scheduling and performing equipment orientations, taking payments, and writing receipts.
  • Responsible for new staff/intern training, in services, and education for other departments/physicians and staff about the benefits of Cardiac Rehab, the referral process, and overview of the program; responsible for ongoing program development, growth, and development of patient education materials.
  • Works collaboratively with other departments to maintain best practice and safest patient care, including regional Cardiac Rehab locations, medical providers, nursing, Biomed, maintenance, environmental services, Wisenbaker Patient Education Center, food service, valet, registration, information services, and Security.
  • Other duties as assigned.

Requirements:

  • 1 – 3 years of experience preferred
  • BLS required
  • ACLS preferred
  • Certified Cardiac Rehabilitation Professional certification (CCRP) required
