Nashville General Hospital is hiring for a Cardiology Care Navigator as a full-time job in Nashville, Tennessee. Shift Monday-Friday 8am-4:30pm
The Cardiology Care Navigator is a non-clinical patient coordination role responsible for helping patients navigate the cardiovascular care process from referral and scheduling through diagnostic testing and follow-up care. This position coordinates cardiac imaging, appointments, referrals, patient communication, and operational work queues to ensure patients receive timely and efficient care.
The Cardiology Care Navigator works closely with cardiologists, physicians, nurses, Radiology, Patient Access, and other healthcare teams to improve patient access, increase completion of cardiac imaging, reduce scheduling delays, close care gaps, and support the growth of cardiovascular services.
Cardiology Care Navigator Roles & Responsibilities:
Patient Navigation & Care Coordination
- Coordinate patients from referral through cardiac imaging and follow-up care.
- Serve as the primary non-clinical contact for scheduling and patient coordination.
- Monitor cardiac CT, MRI, stress echo, PET, SPECT, and other cardiovascular testing.
- Track outstanding orders and appointments to ensure completion.
Imaging & Scheduling Coordination
- Manage cardiac imaging and scheduling work queues.
- Coordinate appointments with Cardiology, Radiology, and other departments.
- Verify orders and coordinate insurance authorizations.
- Reschedule missed or canceled appointments and manage imaging waitlists.
- Identify scheduling delays and same-day opportunities to maximize capacity.
Patient Follow-Up & Communication
- Schedule recommended imaging and follow-up appointments.
- Contact patients regarding appointments, preparation, missed visits, and outstanding testing.
- Coordinate follow-up after emergency department visits and hospitalizations.
- Answer non-clinical questions and route clinical concerns to appropriate staff.
Care Gap & Referral Management
- Manage referral, imaging, recall, hospital follow-up, missed appointment, and waitlist work queues.
- Conduct patient outreach to improve appointment and testing completion.
- Monitor referral conversion and identify barriers to care.
Clinical & Departmental Coordination
- Collaborate with physicians, nurses, Radiology, Patient Access, and other healthcare teams.
- Coordinate patient preparation and clinical instructions with appropriate staff.
- Support communication between patients, providers, and departments.
Reporting & Performance
- Maintain tracking tools, dashboards, and reports for orders, referrals, appointments, and work queues.
- Monitor completion rates, turnaround times, no-shows, referral conversion, and operational barriers.
- Identify opportunities to improve workflows and cardiovascular services.
Documentation & Quality Improvement
- Document patient outreach and coordination activities in the EMR.
- Maintain accurate tracking and reporting tools.
- Participate in quality improvement, workflow redesign, meetings, and special projects.
- Crosstrain to support ambulatory Cardiology operations as needed.
Knowledge, Skills and Abilities
- Excellent organizational and communication skills.
- Strong customer service and interpersonal skills.
- Knowledge of medical terminology, referral management, and insurance authorization processes.
- Ability to manage multiple priorities independently and meet deadlines.
- Strong attention to detail and follow-through.
- Proficiency with Microsoft Office and electronic medical record (EMR) systems.
- Ability to learn and effectively use various healthcare computer systems and software applications.
Minimum Qualifications
- Associate's degree preferred.
- Three years of healthcare experience preferred.
- Proficiency in Microsoft Office, including Excel and Word, is required.
- Proficiency with other computer systems and software applications, with the ability to learn and navigate new systems as needed, is required.
- BLS (Basic Life Support) certification is required prior to the start date.
- Experience in cardiology, ambulatory operations, patient access, scheduling, referral coordination, or care coordination preferred.
- Experience with Cerner or a comparable electronic medical record (EMR) system preferred.
