Job description/Key Responsibilities

  • Personal care support, including medication reminders and personal hygiene.
  • Meal and Fluid preparation
  • Accessing the community, assisting clients with shopping and errands.
  • Travel to and accompaniment to social and medical appointments.
  • End of Life Care
  • Support with discharge from hospital
  • Dementia Care
  • Sit in Care
  • Domestic duties and housekeeping
  • Care provision

Person Specification

  • Good understanding of the care and support needs of the client
  • Ability to use a person-centred approach to undertake the tasks detailed in the client's care and support plan and in the least intrusive way
  • Confident providing input into the care and support plans of clients by regularly feeding back to management
  • Ability to use manual handling equipment safely and correctly.
  • Good communication skills and ability to develop effective working relationships with clients.
  • Listening skills and able to respect the needs of the client.
