Overview

We are seeking a compassionate and motivated Care Coach I to support individuals and families in achieving and maintaining optimal health and wellness. This role focuses on assessing member needs, identifying barriers to care, and guiding members toward appropriate resources and services. The Care Coach I works with moderate autonomy on straightforward to moderately complex assignments and plays a key role in care coordination, health coaching, and ongoing care planning.

This is a patient facing position that emphasizes strong communication, organization, and a passion for improving consumer experiences.

Responsibilities

  • Assess and evaluate member needs, goals, and requirements to support optimal wellness
  • Guide members and families toward appropriate health, wellness, and community resources
  • Identify and help resolve barriers that may hinder effective care
  • Monitor member progress through assessments, data review, and ongoing conversations
  • Develop and maintain active care plans to ensure progress toward desired outcomes
  • Document care activities and member interactions accurately in electronic systems
  • Apply professional standards, regulations, and operating guidelines to daily work
  • Manage assigned work priorities while following established procedures and direction

Required Qualifications

  • Bachelor s degree
  • 2+ years of related experience in care coordination, health coaching, social services, or a related field
  • Proficiency with Microsoft Word and Excel
  • Ability to use multiple electronic applications and systems
  • Exceptional verbal, written, and interpersonal communication skills
  • Valid state driver s license and proof of personal auto insurance
  • Willingness to participate in Tuberculosis (TB) screening as required for patient facing roles
  • Strong passion for improving consumer and patient experiences

Preferred Qualifications

  • Master s degree
  • Experience working with Medicare and Medicaid populations
  • Experience documenting in electronic case management or clinical systems
  • Background in health promotion, wellness, or coaching programs
  • Knowledge of community health organizations and social service resources
