Overview
We are seeking a compassionate and motivated
Care Coach I to support members in achieving and maintaining optimal health and wellness outcomes. This role serves as an advocate, educator, and care coordinator, helping members navigate healthcare resources, overcome barriers to care, and actively participate in their healthcare journey.
The ideal candidate has experience working with diverse populations, strong communication skills, and a passion for improving health outcomes through coaching, care coordination, and member engagement.
Responsibilities
Care Coordination & Member Support
- Assess member needs and develop individualized care plans
- Guide members and families toward healthcare, community, and social service resources
- Identify and address barriers that may impact health outcomes, treatment adherence, or access to care
- Support members in achieving healthcare goals through education, coaching, and ongoing engagement
- Monitor member progress through assessments, data review, and direct communication
- Collaborate with healthcare providers, caregivers, and community partners to coordinate services
Health Coaching & Wellness
- Utilize coaching techniques to promote behavior change and healthy lifestyle choices
- Support chronic condition management and preventive health initiatives
- Encourage member participation in wellness and health promotion programs
- Assist members in understanding available healthcare benefits and resources
Documentation & Case Management
- Document member interactions, care plans, and progress accurately in electronic systems
- Maintain case notes and records across multiple platforms
- Track outcomes and support continuous improvement of care management activities
- Follow established policies, procedures, and professional standards
Member Advocacy
- Serve as a resource and advocate for members and families
- Promote positive healthcare experiences and improved care outcomes
- Communicate effectively with members, providers, and internal stakeholders
- Support initiatives focused on consumer satisfaction and quality improvement
Required Qualifications
- Bachelor's degree
- Minimum of 2 years of related experience in healthcare, care coordination, case management, social services, wellness coaching, or a related field
- Strong knowledge of Microsoft Word and Excel
- Excellent verbal and written communication skills
- Strong interpersonal and relationship-building abilities
- Ability to learn and utilize healthcare technology and electronic systems
- Experience working with multiple software applications and databases
- Valid state driver's license
- Reliable personal vehicle
- Proof of personal automobile liability insurance with minimum coverage of:
- $100,000 bodily injury per person
- $300,000 bodily injury per accident
- $100,000 property damage liability
- Ability to complete required Tuberculosis (TB) screening
- Passion for improving consumer experiences and health outcomes
Preferred Qualifications
- Master's degree
- Experience working with Medicare and Medicaid populations
- Experience documenting electronic case notes and working across multiple healthcare systems
- Experience with health coaching, wellness programs, or care management
- Knowledge of community health agencies, social service organizations, and local community resources
- Experience supporting vulnerable or complex-care populations
