Care Connector Bilingual

This role is hybrid, working (4) days in office and one (1) day remote. You must have access to a reliable high-speed internet connection to support daily job responsibilities. A minimum bandwidth of 50 Mbps download and 5 Mbps upload is required.

Responsibilities:


The Care Connector (CC) is responsible for supporting the daily operations of integrated care management program interventions. The CC is assigned members with Care Gaps/HEDIS related health conditions and assists them in accessing care through Plan benefits and community resources. Under the direction of Clinical staff, the Care Connector will provide members with educational materials and carry out strategies to increase health care adherence and reduce barriers to care. The CC is responsible for a caseload of members with low-risk health concerns who require assistance with navigating health systems.


Education/ Experience:


+ High School/GED.

+ Must be bilingual, demonstrating the ability to speak both English and one of two target languages - Spanish or Amharic. Must be able to perform the responsibilities of the position either in English or in another language. Languages required are determined by business need.

+ 1 to 3 years' experience in a health-related setting.


Other Skills:


+ Proficient PC skills in a Windows based environment including word processing, spread sheets and working in database programs.

+ Working knowledge of basic healthcare terminology; especially for maternity population.

+ Working knowledge of Plan Benefits and Services is preferred.

+ Excellent telephone/customer service skills.

+ Proven ability to keep accurate and timely records and documentation according to established processes.

Salary/Pay Range


The range displayed in this job posting reflects the minimum and maximum for new hire salaries for the position in the Washington, DC area. Within the range, individual pay is determined by additional factors, including, without limitation, job-related skills, experience, and relevant education, certifications, or training.


AmeriHealth Caritas associates are eligible to participate in our annual incentive program and will also receive our benefits package, consisting of medical, vision, dental, life insurance, disability insurance, 401(k), paid time off and more.


The targeted hiring range for this role is expected to be between $27.60 (Min) and $37.60(Max) per hour.
