Care Coordinator / Case Manager (Full Time)

About the role
Rising Star Home Care Services is a Maryland licensed Medicaid home care agency serving clients across the state under the Community First Choice and Community Personal Assistance Services programs.
We are not trying to be another agency that fills shifts and bills hours. We intend to be the best home care agency in Maryland. Not the biggest, the best. The one that support planners call first because they know the referral will be handled. The one families recommend to their neighbors. The one caregivers do not leave.
That is a real goal and we are honest that we are not there yet. We are a small, independently owned agency that has the license, the programs, and the clinical leadership in place. What we need now is the person who builds the operation underneath it.
If you have worked as a support planner, case manager, or service coordinator, you already know the hard part of this work. A plan of service is written, hours are authorized, and then someone has to make sure what happens in the home actually matches the paper. You have also probably spent years watching agencies do that badly. This is your chance to run it the way you always thought it should be run.
How we work
You will not be managed day to day. You will be given the outcomes we care about and the authority to hit them.
If you see that our onboarding is losing caregivers in week two, fix onboarding. If our support planner relationships are thin in one part of the state, go build them. If the schedule is being held together by memory, build the system that replaces the memory. Bring us the problem and your plan, not the problem alone.
Our team is small enough that your work is visible and your decisions stick. Nobody here waits three weeks for a decision from someone who has never met the client. As the agency grows, this role grows with it, and we would much rather promote the person who built the operation than hire over them.
The schedule is 10:00 AM to 4:00 PM, Monday through Friday. That is deliberate. It is a full week with a full seat at the table, and it still leaves your mornings and your evenings alone. If you have been carrying a 45 hour week to do work like this, that difference is the point.
What you will own
- The plan of care. Confirm that authorized services are being delivered as written, and escalate when they are not.
- Client homes. Visit regularly, introduce new caregivers, walk the home, and talk to families about how things are actually going.
- Support planner relationships. Weekly check ins, clean documentation, and a reputation that makes us the agency they refer to first.
- The master schedule. Every client, every authorized hour, matched to a caregiver who is a fit.
- Coverage. Fill open shifts and find replacements when someone calls out.
- Caregiver recruitment and onboarding. Screen applicants, run in office application meetings, and get files complete before anyone starts.
- Compliance in LTSS. Correct missed clock ins and clock outs on the day they are flagged.
- Credentials. Keep caregiver training, certifications, and portal access current, and chase expirations before they lapse.
- Client assessments, scheduled and coordinated with our nursing team.
- The systems themselves. Improve how all of the above gets done, and write it down so it survives you being on vacation.
What we need from you
- Experience in a Medicaid funded service. Support planning, case management, service coordination, home care, home health, or senior care all count. This is not a role we can train from scratch.
- Someone genuinely autonomous. You set your own priorities, you close your own open items, and you do not need to be chased. This is the single most important thing on this list.
- The instinct to build rather than cope. We would rather hear how you fixed the process than how you survived it.
- Comfort with scheduling systems and electronic visit verification. If you have used Maryland LTSS, say so in your application.
- Valid driver's license, reliable vehicle, and willingness to drive to client homes across your assigned territory.
- Clear, warm communication with caregivers, families, and support planners.
- Ability to pass a criminal background check.
Preferred
- Bachelor's degree in social work, human services, psychology, public health, nursing, gerontology, or a related field. This is considered when setting your starting rate.
- Prior work at a state agency, a supports planning agency, an Area Agency on Aging, or another state contracted program. You already know the people on the other end of the phone, and that is worth a great deal to us.
- Familiarity with OHCQ standards and Maryland Medicaid CFC or CPAS program rules.
- CNA or GNA certification. Also considered when setting your starting rate.
- CareerPlug or another applicant tracking system.
What success looks like in your first year
- No client goes without a caregiver, and no authorized hour goes uncovered.
- Services delivered in the home match the plan of service on file, every time.
- New caregivers start with complete files, and nobody starts before meeting their client in person.
- Missed punches are corrected on the day they are found.
- Caregiver training and certifications never lapse.
- Caregiver turnover goes down because people want to work here.
- Support planners across the state know your name and send us their referrals.
