Care Coordinator – CNA, MA, LPN, RN

As a Care Coordinator with UltaCare Medical, you will become a valued member of the facility team providing care for residents in a variety of post-acute settings. Our opportunity is designed for those who have strong organizational skills, are highly dependable, and are open to learning and growing professionally.
Essential Functions and Responsibilities:
- Daily workflow management. Create and manage daily rounding lists and schedules for routine visits.
- Documentation support. Assist with researching health information in the EHR and providing scribe services to rounding providers.
- Quality Improvement. Support and assist with data collection to improve practice and outcomes.
- Team Support. Provide support in other areas of UltaCare as directed by Regional Director.
- Obtain insurance.
Financial Rewards and Benefits:
- Annual compensation ranges from $50,000 - $55,000.
- Weekdays only schedule, no nights or weekends, holidays off.

Qualifications:
- Current TN license.
- Ability to efficiently navigate multiple EHR systems.
- Strong organizational skills.
- Strong written and verbal communication skills.
- Prefer experience in post-acute or long-term care setting.
