United Methodist Communities in Camden County, a 5-Star full-service senior living community that is a model in person-centered care community, located in beautiful Collingswood, NJ is seeking a Care Coordinator.
Care Coordinator Requirements:
- Proficient in Microsoft Office applications
- 1 year of experience in case management marketing
- 2 years of experience in case management marketing preferred
- One of the following licenses or a bachelor's degree in a related field from an accredited college or university:
What You'll Do:
- Gather and evaluate referral information, review submitted materials, and/or assess potential residents for admission
- Make admission decisions and/or collaborate with nursing staff on healthcare admission determinations
- Build and maintain a referral network, including conducting onsite visits as needed
- Facilitate the admission process into healthcare, ensuring all required paperwork is completed and processed in a timely manner
- Verify Medicare coverage and/or other insurance benefits
- Participate in care conferences and other required meetings
- Follow up with residents post-discharge to ensure well-being; complete medication reconciliation and psychosocial assessments
- Support post-acute discharge care in the home setting
- Ensure departmental reports are completed accurately and on time
