Coordinates client services, staffing, scheduling, and communication to ensure continuity of care across home care and community-based lines. Serves as a central point of contact for clients, care professionals, and internal teams.
Key Responsibilities:
- Manage client intake, scheduling, and service coordination
- Communicate with clients, families and care professionals
- Track authorizations, start-of-care timelines, and service changes
- Support documentation accuracy and compliance
- Escalate concerns to leadership as needed
Required Qualifications:
- Experience in healthcare coordination, home care, or human services preferred
- Strong organizational, communication, and problem-solving skills
- Ability to manage multiple priorities in a fast-paced environment
Additional Requirements:
- Ability to travel up to 30 miles within assigned service area (mileage reimburse when applicable)
- Valid driver's license and reliable transportation
- Background check and credential verification
