Power Health Colorado (PHCO) is seeking a highly organized, proactive, and patient-focused Care Coordinator to support the continuity of care for both Personal Injury (PI) and Workers’ Compensation (WC) patients. This is a fast-paced role that requires strong critical thinking, multitasking, customer service, and problem-solving skills.
This position is ideal for someone who thrives in a team-based healthcare environment, takes ownership of patient outcomes, and understands the importance of urgency, communication, and follow-through.
At PHCO, we are building an integrated healthcare organization focused on delivering exceptional care for injured patients through collaboration, communication, and operational excellence. Our team is committed to creating a supportive, fast-moving environment where employees can grow professionally while making a meaningful impact on patient outcomes.
