About the Role:
FirstLight Home Care in Simi Valley, CA is looking for a compassionate and organized Care Coordinator to join our growing team! In this rewarding role, you'll be the vital link between our clients, their families, and our caregiving staff — making a real difference in people's lives every single day.
Responsibilities:
- Coordinate and schedule caregivers to match client needs, preferences, and care plans
- Serve as a point of contact for clients, families, and caregiving staff
- Conduct client intake calls and assist with onboarding new home care clients
- Monitor and manage caregiver schedules to ensure consistent, uninterrupted care coverage
- Respond promptly to scheduling changes, call-outs, and urgent client needs (may need to cover shifts)
- Maintain accurate client and caregiver records in the scheduling and care management system
- Collaborate with the care team to ensure high-quality, client-centered home care delivery
Requirements:
- Prior experience in home care, healthcare coordination, or a related field preferred
- Home Care Aide Certificate required
- Strong organizational and multitasking skills in a fast-paced environment
- Excellent verbal and written communication skills
- Compassionate, client-focused attitude with a passion for serving seniors and families
- Proficiency with scheduling software, CRM systems, or willingness to learn
- Ability to problem-solve quickly and remain calm under pressure
- High school diploma or equivalent required; associate's or bachelor's degree a plus
