We are looking for a passionate clinician to join our team as a Care Coordinator, serving as a trusted clinical resource to hospitals, skilled nursing facilities, rehabilitation centers, physicians, and other healthcare partners throughout the community. This is not traditional sales, this is clinical leadership, patient advocacy, and relationship-building at its finest.

Qualifications
Must be a graduate of an approved school of nursing or therapy.
Education and experience, preferred
Preferred Clinical Backgrounds
Registered Nurse (RN)
Licensed Practical Nurse (LPN)
Physical Therapist (PT)
Physical Therapist Assistant (PTA)
Occupational Therapist (OT)
Certified Occupational Therapy Assistant (COTA)
Three (3) years of field experience is preferred.
Previous experience in home health or healthcare sales is preferred.
