Job Summary:Under the direction of the ECM Program Director, the Care Coordinator & Housing Navigator provides non-clinical care coordination and housing navigation services to members enrolled in California's Enhanced Care Management (ECM) program. This position supports whole-person care by assisting members with healthcare navigation, connecting them to community and housing resources, and promoting engagement in their care plans.The role works as part of a Community-Based Care Management Entity (CB-CME), collaborating closely with clinical team members to support care plan implementation, improve access to services, and address social determinants of health, including housing instability.RequirementsMinimum & Preferred Qualifications:Education/TrainingMinimum: High school diploma or equivalent.Preferred: Medical Assistant (MA), Certified Nursing Assistant (CAN), Associate or Bachelor's degree in a related field.ExperienceMinimum: Two (2) years of customer service or administrative experience.Preferred: Experience in healthcare, managed care, or care coordination. Experience with referral processes, utilization management, or community-based services. Familiarity with ICD-10/CPT Coding.Any combination of educational and work experience that would be equivalent to the stated minimum requirements would qualify for consideration of this position.Certification(s)Maintains a valid California Driver's License and current automobile insurance when using a personal vehicle for Company business.Skills, Knowledge & AbilitiesStrong interpersonal and relationship-building skills.Effective communication and conflict resolution skills.Ability to engage members and providers across in-person and telehealth settings.Cultural competence and sensitivity to diverse populations.Basic understanding of healthcare navigation and behavioral health conditions.Awareness of implicit bias and commitment to equitable care.Strong organizational, time management, and data entry skills.Proficiency in Microsoft Office and general computer applications.Ability to work independently within a team-based environment.Bilingual (English/Spanish) preferred.Typing of 45 words per minute.Physical, Mental & Environmental RequirementsThe physical, mental, and environmental demands described here are representative of those required to successfully perform the essential functions of this position. Primarily field-based role requiring regular travel between Company locations and other business sites as required; including standing, walking, and driving.
