Care Coordinator II
Contract Type / Duration: Contract | 6 Months with Potential Extension
Location: Remote Kentucky Residents Only
Target Start Date: September 21, 2026
Pay Rate: $20 23/hour W-2, depending on certifications
Work Schedule
Training: 8:00 AM 4:30 PM EST/CST
- 2 weeks of classroom training followed by 1 week of one-on-one training
- Cameras required during training
- No time off during the training period
Regular Schedule: 8-hour shifts beginning around 9:00 or 9:30 AM and potentially ending as late as 7:00 PM EST/CST. Shifts may flex based on department needs.
Job Summary
We are seeking a compassionate and dependable Care Coordinator II to support member outreach, care coordination, and preventive health initiatives for members throughout Kentucky.
This role focuses heavily on outbound member engagement, helping members access healthcare services, scheduling provider appointments, connecting members with community resources, and closing preventive care gaps.
The ideal candidate has healthcare or community-based experience and is comfortable building rapport with members over the phone while documenting information accurately across multiple systems.
Key Responsibilities
- Conduct outbound member outreach and respond to inbound calls
- Educate members about available healthcare services and resources
- Schedule provider appointments and coordinate follow-up care
- Assist members with community and social determinant of health (SDOH) resources
- Document member interactions accurately while speaking with members
- Support preventive care initiatives, including well-child visits, postpartum care, A1C testing, and breast cancer screenings
- Maintain accurate documentation across multiple systems
- Meet established productivity, documentation quality, and member engagement goals
- Participate in virtual team meetings with cameras on
- Attend quarterly in-person staff meetings in Louisville, KY, when requested

Required Qualifications
- High school diploma or equivalent
- 1 2 years of related experience
- Outbound member outreach experience
- Provider or appointment scheduling experience
- Healthcare, medical/clinical, or community-based support experience
- Strong computer and data-entry skills
- Ability to work independently in a remote environment
- Strong communication and active listening skills
- Ability to quickly build rapport with members
- Reliable, adaptable, and comfortable working in a fast-paced environment
Preferred Qualifications
- Certified Community Health Worker (CHW)
- LPN
- Community resource knowledge
- Care coordination experience
- SDOH resource coordination
- Previous remote work experience
Additional Requirements
Candidates must reside in Kentucky and have an updated smartphone capable of supporting current security requirements (iOS 17+ for Apple or Android 13+). A basic computer skills assessment is required.
