INTRODUCTION

We are seeking a friendly, organized, and dependable Care Coordinator to join our team at our Jordan Valley, Oregon Health Neighborhood Center.


The Care Coordinator is responsible for coordinating patient care across providers, services, and community resources to help ensure patients receive timely, appropriate, and well-organized healthcare. This role serves as a key point of contact for patients, families, healthcare providers, and internal clinical teams, helping patients navigate appointments, referrals, treatment plans, insurance requirements, and barriers to care.

The successful candidate will have strong communication, organizational, and customer service skills; the ability to manage difficult or emotionally challenging situations with empathy and professionalism; and a commitment to responding promptly and respectfully to the needs of patients and the care team.

PRIMARY ROLE AND RESPONSIBILITIES 

•         Serve as a primary point of contact for patients and caregivers regarding care coordination needs.

•         Schedule and coordinate medical appointments, specialist referrals, diagnostic testing, procedures, and follow-up care.

•         Track referrals and follow up to help ensure patients complete recommended services.

•         Assist patients in understanding care plans, provider instructions, and next steps within the scope of the coordinator role.

•         Identify barriers to care, including transportation, financial, insurance, language, social, and access-related challenges, and connect patients with appropriate resources.

•         Coordinate transitions of care following emergency department visits, hospitalizations, procedures, or changes in level of care.

•         Communicate relevant patient information to members of the care team in a timely and professional manner.

•         Maintain accurate and timely documentation of patient interactions, referrals, appointments, care coordination activities, and outcomes in the electronic health record (EHR) or other designated systems.

•         Monitor assigned patient populations and follow up on outstanding care needs, appointments, tests, and referrals.

•         Assist with obtaining medical records, prior authorizations, insurance information, and other documentation as required.

•         Collaborate with physicians, nurses, social workers, case managers, behavioral health professionals, and other members of the interdisciplinary care team.

•         Educate patients about available healthcare and community resources without providing medical advice outside the scope of the position.

•         Escalate clinical questions, changes in patient condition, and urgent concerns to the appropriate licensed clinical professional.

•         Protect patient confidentiality and comply with HIPAA, organizational policies, and applicable healthcare regulations.

•         Participate in care team meetings, case reviews, quality improvement initiatives, and other departmental activities as assigned.

QUALIFICATIONS:

•         Education: High school diploma or equivalent

•         Experience: Minimum of three (3) years of experience working in the healthcare field

•         Skills: Strong computer skills, including proficiency with web-based applications and electronic health records.

•         Languages:  English mandatory, bilingual (Spanish) preferred.

•         Knowledge: Working knowledge of healthcare delivery systems, including medical, behavioral health, social service, referral, and community resource systems

•         Self-starter with strong organizational skills and ability to manage multiple priorities.

•         Experience working with diverse, underserved, and/or rural populations.

•         Excellent interpersonal communication, facilitation, and writing skills.

•         Ability to work both independently and as part of interdisciplinary and cross-system teams.

•         Must have a current driver’s license, clean driving record, and reliable transportation for travel to community and clinical sites.

•         Must pass a multi-state criminal background check.
